Blood Pressure: Ages 0-60  135/85… Ages 61+  140/85.  No blood pressure medicine allowed.

Cholesterol: Total 205 – HDL < or = to 5.0  OR  Total 260 – HDL < or = to 4.5

Family History: No cardiovascular disease or cancer in parents prior to age 60

Tobacco:  No tobacco use for 5 years EXCEPT can smoke up to 52 cigars per year.

Driving Record: 1 violation in the past 3 years.  No DUI, reckless revocation or suspension in the last 5 years.

#2 Health Class = Preferred

Blood Pressure: Ages 0-60  140/85… Ages 61+  150/85 OR #1 Health Class = Preferred Plus BP readings with medication.

Cholesterol: Total 235 – HDL < or = to 6.0  OR  Total 280 – HDL < or = to 5.5

Family History: No cardiovascular or cancer death in parents prior to age 60

Tobacco:  No tobacco use for 3 years EXCEPT can smoke up to 52 cigars per year.

Driving Record: 2 violations in the past 3 years.  No DUI, reckless revocation or suspension in the last 5 years.

#3 Health Class = Standard Plus

Blood Pressure: Ages 0-60  145/88… Ages 61+  155/88 OR #2 Health Class = Preferred BP readings with medication.

Cholesterol: Total 250 – HDL < or = to 7.0  OR  Total 280 – HDL < or = to 6.5

Family History: Can have one parent die of cardiovascular disease or cancer prior to age 60

Blood Pressure: Through Age 70 – Average from current medical exam and history readings with in the last year do not exceed 135/85 through age 60 or 145/85 for ages 61-70.  Treated blood pressure must have been controlled for 1 year.

Cholesterol: Through Age 70 – Total Cholesterol not greater than 275 including treated cholesterol.  Cholesterol/HDL Ratios 4.5 or less. OVER Age 70 – Total Cholesterol may not be lower than 130 untreated and may not exceed 275 with or without treatment. Cholesterol/HDL Ratio may not exceed 4.5.

Family History: Through Age 70 – No history of or death from cancer, heart disease, or any cardiac-related condition, of either natural parent or sibling prior to age 60. Waived if the applicant is actual age 60 or older unless both natural parents died from one of the same preceding impairments prior to age 60.  Family history cancers are limited to those types that clearly demonstrate a genetic predisposition, i.e. breast, colon, prostate, ovarian, melanoma, lung cancer.  OVER Age 70 – None.

Tobacco:  All Ages – No nicotine use for the past 5 years (urine nicotine negative). Will allow up to 6 celebratory cigars over the past 12 months if usage is admitted on the application and/or medical examination and the current urine specimen is negative for nicotine

Driving Record: All Ages – Not available if two or more moving violations in the last three years or any DUI or reckless driving in the last five years

#2 Health Class = Preferred

Blood Pressure: Through Age 70 – Average from current medical exam ad history readings within the last year do not exceed 140/90 through age 60 or 150/90 for ages 61-70. Treated blood pressure must have been controlled for one year.  OVER Age 70 – 
Average from exam and readings within the last year may not exceed 160/95. Treated blood pressure must have been controlled for one year.

Cholesterol: Through Age 70 – Total Cholesterol not greater than 275 including treated cholesterol and Cholesterol/HDL Ration 5.5 or less. OVER Age 70 – Total Cholesterol may not be lower than 130 untreated and may not exceed 275 with or without treatment. Cholesterol/HDL Ratio may not exceed 5.5
