Top 3 Health Classes

#1 Health Class = Elite Plus

#2 Health Class =  Preferred Plus

#3 Health Class = Standard Plus

#1 Health Class = Elite Plus

Blood Pressure: Ages 54 & under – 130/80 or less.  Ages 55 & over – 135/82 or less.  No blood pressure medication within the last 12 months

Cholesterol: Ages 54 & under – 200 or less.  Ages 55-69 –  210 or less.  Ages 70 & over – 150-210.  HDL Ratio 4.5 or less for all ages.  Current medication okay if acceptable level maintained for at least 12 months.

Family History: Ages 65 & under – No death from cardiovascular disease of cancer in parent prior to age 60 or in sibling prior to age 65.  Some cancers may qualify.  A wellness consideration MAY be allowed for proposed insureds ages 40 – 65 to offset one early family history death due to cardiovascular disease or cancer.

Tobacco: No nicotine use  of any form in the past 60 months plus a negative nicotine urine test.

Driving Record: No DWI convictions within past 5 years. Maximum of 1 moving violation  within past 3 years.

#2 Health Class = Preferred Plus

Blood Pressure: Age 54 & under – 140/85 or less.  Age 55 & over – 140/90 or less.  No blood pressure medication within the last 12 months.

Cholesterol: Age 54 & under – 220 or less and Ratio 5.0  or less.  Age 55 & over – 240 or less and a HDL Ratio of 5.0 or less.   OR 260 or less a HDL Ratio of 4.5 or less.  Current medication okay if acceptable level maintained for at least 12 months.

Family History: Ages 65 & younger –  No death from cardiovascular disease or cancer  in a parent prior to age 60 or in a sibling prior to age  65.  Some cancers MAY qualify.  A “wellness” consideration may be allowed for proposed insureds age 50-65 to offset one early family history death due to cardiovascular disease or cancer.

Tobacco: No use of nicotine in any form for past 60 months and negative nicotine urine test. A celebratory cigar (limited to 4 per year) with negative nicotine urine test qualifies.

Driving Record: No DWI convictions within past 5 years. Maximum of 2 moving violations within past 3 years.

#3 Health Class = Standard Plus

Blood Pressure: Age 54 & under – 140/90 or less.  Age 55 to 69 – 150/90 or less.  Ages 70 & over –  155/90 or less. Current medication acceptable for all ages.

Cholesterol: Age 54 & under –  260 or less and a HDL Ratio of 6.0 or less.  Age 55 to 69 –  280 or less and a HDL Ratio of 6.5 or less.  Age 70 & over –  150 – 300 and a HDL Ratio of  7.0 or less.   Current medication is acceptable.

Family History: Ages 65 & Under – No death from cardiovascular disease or cancer in immediate family member (parent or siblings) prior to age 60.  Some cancers MAY qualify.   A “wellness” consideration may be allowed for proposed insureds age 40-65 to offset one early family history death due to cardiovascular disease or cancer.

Tobacco: No cigarettes or tobacco substitutes in the past 24 months and negative nicotine urine test.

Driving Record: No DWI convictions within past 5 years. Maximum 2 moving violations within past 3 years.

Blood Pressure: Currently well controlled with or without treatment, with the average readings in the past two years not greater than 136/86.

Cholesterol: 120-300 with or without treatment; CHOL/HDL Ratio may not exceed 4.5 with or without treatment.

Family History: Cardiovascular Disease: No cardiovascular disease in either parent or sibling before age 60. A family history of cardiovascular disease is not a consideration for applicants over 70 who don’t use tobacco.

Tobacco: No use of tobacco or nicotine-based products in the past 36 months. One celebratory cigar allowed per month with urine specimen negative for nicotine.

Driving Record: No more than 2 moving violations in last 3 years. No DWI, DUI, reckless driving, license revocation or suspension in last 5 years.

#2 Health Class = Preferred

Blood Pressure: Currently well controlled with or without treatment, with the average readings in the past two years not greater than 146/90

Cholesterol: 120-300 with or without treatment; CHOL/HDL Ratio may not exceed 5.5 with or without treatment

Family History: Cardiovascular Disease: No cardiovascular death in either parent before age 60.  A family history of cardiovascular disease is not a consideration for applicants over age 70 who don’t use tobacco.

Tobacco: No use of tobacco or nicotine-based products in the last 24 months. One celebratory cigar allowed per month with urine specimen negative for nicotine.

Driving Record: No more than 2 moving violations in last 3 years. No DWI, DUI, reckless driving, license revocation or suspension in last 5 years.

#3 Health Class = Standard Plus

Blood Pressure: Currently well controlled with or without treatment, with the average readings in the past two years not greater than 152/92.

Cholesterol: 120-300 with or without treatment; CHOL/HDL Ratio may not exceed 6.5 with or without treatment.

Family History: Cardiovascular Disease: No cardiovascular death of more than one parent before age 60. A family history of cardiovascular disease is not a consideration for applicants over age 70 who don’t use tobacco.

Tobacco: No use of tobacco or nicotine-based products in the last 12 months. One celebratory cigar allowed per month with urine  specimen negative for nicotine.

Driving Record: No more than 3 moving violations in last 3 years. No DWI, DUI, reckless driving, license revocation or suspension in last 3 years.
