Top 3 Health Classes

#1 Health Class = Preferred Best

#2 Health Class =  Preferred

#3 Health Class = Non-smoker Plus

#1 Health Class = Preferred Best

Blood Pressure: Up to 130/80 for ages up to 49.  Up to 135/85 for ages 50 and over.  Without medication.

Cholesterol: Cholesterol/HDL ratio 5 or less. With or without medication.

Family History: No death of parents or siblings from Coronary Artery Disease, Cerebrovascular Disease or cancer prior to age 60.  Cancer limited to those with a high family risk – breast, colon, melanoma, ovarian, pancreas, prostate and stomach.

Tobacco: No tobacco or nicotine use within last 5 years.

Driving Record: No DWI or reckless driving within the last 5 years.  No license suspension within the last 3 years.  No more than 1 citation for moving violation or motor vehicle accident within the last 3 years.

#2 Health Class = Preferred Plus

Blood Pressure: Up to 135/85 for ages up to 49.  Up to 140/90 for ages 50 and over.  With or without medication.

Cholesterol: Cholesterol/HDL ratio 6 or less.  With or without medication.

Family History: No death of more than one parent from Coronary Artery Disease, Cerebrovascular Disease, or cancer prior to age 60.  Cancer limited to those with a high family risk – breast, colon, melanoma, ovarian, pancreas,  prostate and stomach.

Tobacco: No tobacco or nicotine use within last 3 years.

Driving Record: No DWI or reckless driving within the last 5 years.  No license suspension within the last 3 years.  No more than  2 citations for moving violation or motor vehicle accidents within the last 3 years.

#3 Health Class = Standard Plus

Blood Pressure: Up to 140/90 for ages up to 49.  Up to 145/90 for ages 50 and over.  With or without medication.

Cholesterol: Cholesterol/HDL ratio 7 or less.  With or without medication.

Family History: No Specific Criteria.

Tobacco: No cigarettes within last 12 months.  May smoke cigars, pipe or chew tobacco.

Driving Record: Actual driving history will be evaluated to determine qualification for this class.

Top 2 Health Classes

Blood Pressure: Up to age 69 – Treated or untreated hypertension with a 12 month average of 130/80 or lower.  Age 70 and up –  Treated or untreated hypertension with a 12 month average of 140/90 or lower

Cholesterol: Minimum untreated cholesterol – 125.  Maximum cholesterol – 300.   Up to age 69 – Treated or untreated readings –  – Cholesterol/HDL ration < or = 5.0.   Age 70 and up –  Treated or untreated readings – Cholesterol/HDL ration < or = 5.5.

Family History: Up to age 69 – No deaths of parents or siblings prior to age 65 due to cardiovascular disease.  Age 70 and up – Disregard family history of cardiovascular disease.

Tobacco: No tobacco or nicotine products in the past 36 months. Occasional cigar use of up to 12  cigars a year with a urinalysis negative for nicotine.

Driving Record: No more than 3 moving violations in the past 3 years. No DUI or license suspension in past 5 years.

#2 Health Class = Preferred

Blood Pressure: Up to age 69 – Treated or untreated hypertension with a 12 month average of 140/90 or lower.   Age 70 and up –  Treated and untreated hypertension with a 12 month average of 155/90 or lower.

Cholesterol: Minimum untreated cholesterol – 125.  Maximum cholesterol – 300.  Up to age 69 – Treated or untreated readings Cholesterol/HDL ration< or = to 6.0.   Age 70 and up – Treated or untreated readings Cholesterol HDL/ration < or = to 7.0.

Family History: Up to age 69 – No deaths of parents or sibling prior to age 60 due to cardiovascular disease.  Age 70 and up – Disregard family history of cardiovascular disease.

Tobacco: No tobacco or nicotine in any form in the past 24 months.  Occasional cigar use of up to 24 cigars a year with a urinalysis  negative for nicotine.

Driving Record: No more than 3 moving violations in the past 3 years.  No DUI or license suspension in past 5 years.
