Banner Life – Sample Underwriting Guidelines

***TEMPORARY UNDERWRITING GUIDELINES DUE TO COVID-19***

COVID-19 cases will be postponed for at least 30 days after full recovery.  Reconsideration depends on the severity of the disease and current health status.

Banner Life Insurance Age & Risk:

Applicants in the below age ranges whose assessed risk class exceeds rated table rating parameters AND the risk profile includes any of the underlying chronic conditions listed below will be postponed for a minimum of 6 months.

  • Ages 50-64 and risk profile exceeds Table 6
  • Ages 65 -70 and risk profile exceeds Table 4
  • Ages 71+ and risk profile exceeds Standard
Banner Underlying chronic conditions:
  • Cancer: Advanced Stage |  Incomplete treatment |  Organ of origin: lung, pancreas, brain, multiple primaries,
    melanoma, incomplete resection, recurrence, ongoing chemotherapy, etc.
  • Cardiovascular / Cerebrovascular disease: Arrhythmia including Atrial fibrillation, Anticoagulant use |  Cardiomyopathies |  Coronary  |artery disease (CAD) |  Stroke, TIA or documented cognitive impairment |  Valvular disease | Other rateable cardiovascular diseases.
  • Dementia
  • Diabetes (type 2; T2DM): With end-organ damage (excluding retinopathy), history, hypoglycemia, compliance/control issues, insulin therapy, young age diagnosis (< age 30)/long duration.
Banner Underlying Medical Conditions (include but are not limited to):
  • Frailty – Includes mobility issues: current, functionally impairing, recovery not expected
  • Immunosuppression –  Current chemotherapy |  History of recurrent infections |  Immunosuppression for complex systemic disease (e.g. rheumatoid arthritis).
  • Kidney Disease – Chronic Kidney Disease | Cystic kidney disease |  Marked proteinuria.
  • Lung disease –  Asthma (moderate or severe) – Chronic Obstructive Pulmonary Disease (COPD) |  Recurrent bronchitis/pneumonia | Other rateable lung diseases
Banner Foreign Travel:

any planned foreign travel will be postponed for a period of 30 days following the applicant’s return from their trip.

#1 BANNER PREFERRED PLUS HEALTH RATING

Blood Pressure – 
Currently well controlled with or without treatment, with the average readings in the past two years not greater than 136/86

Cholesterol –
120-300 with or without treatment; CHOL/HDL Ratio may not exceed 4.5 with or without treatment

Family History – 
Cardiovascular Disease: No cardiovascular disease in either parent or sibling before age 60. A family history of cardiovascular disease is not a consideration for applicants over 70 who don’t use tobacco.

Tobacco Definition – 
No use of tobacco or nicotine-based products in the past 36 months. One celebratory cigar allowed per month with HO specimen negative for nicotine.

Motor Vehicle Report –
No more than 2 moving violations in the last 3 years. No  DWI, DUI, reckless driving, license revocation, or suspension in the last 5 years.

Residence/Citizenship –
Must be a permanent U.S. resident for the past 3 years and either a U.S. citizen or have permanent Visa or Green Card.


#2 BANNER PREFERRED HEALTH CLASS

Blood Pressure –
Currently well controlled with or without treatment, with the average readings in the past two years not greater than 146/90

Cholesterol – 
120-300 with or without treatment; CHOL/HDL Ratio may not exceed 5.5 with or without treatment

Family History –
Cardiovascular Disease: No cardiovascular death in either parent before age 60. A family history of cardiovascular disease is not a consideration for applicants over age 70 who don’t use tobacco.

Tobacco Definition –
 No use of tobacco or nicotine-based products in the last 24 months. One celebratory cigar allowed per month with HO specimen negative for nicotine.

Motor Vehicle Report –
No more than 2 moving violations in the last 3 years. No DWI, DUI, reckless driving, license revocation, or suspension in the last 5 years.

Residence/Citizenship –
Must be a permanent U.S. resident for the past 3 years and either a U.S. citizen or have permanent Visa or Green Card.


#3 BANNER STANDARD PLUS HEALTH RATING

Blood Pressure –
Currently well controlled with or without treatment, with the average readings in the past two years not greater than 152/92

Cholesterol –
120-300 with or without treatment; CHOL/HDL Ratio may not exceed 6.5 with or without treatment

Family History –
Cardiovascular Disease: No cardiovascular death of more than one parent before age 60.  A family history of cardiovascular disease is not a consideration for applicants over age 70 who don’t use tobacco.

Tobacco Definition –
No use of tobacco or nicotine-based products in the last 12 months. One celebratory cigar allowed per month with HO specimen negative for nicotine.

Motor Vehicle Report –
No more than 3 moving violations in the last 3 years. No DWI, DUI, reckless driving, license revocation, or suspension in the last 3 years.

Residence/Citizenship –
Must be a permanent U.S. resident for the past 2 years and either a U.S. citizen or have permanent Visa or Green Card.


#4 BANNER STANDARD HEALTH CLASS

Blood Pressure – 
Currently well controlled with or without treatment, with the average readings in the past two years not greater than 156/94

Cholesterol – 
120-300 with or without treatment; CHOL/HDL Ratio may not exceed 8.0 with or without treatment

Family History –
Cardiovascular Disease: No cardiovascular death of more than one parent before age 60. A family history of cardiovascular disease is not a consideration for applicants over age 70 who don’t use tobacco.

Tobacco Definition – 
No use of tobacco or nicotine-based products in the last 12 months. One celebratory cigar allowed per month with HO specimen negative for nicotine.

Motor Vehicle Report –
No more than 4 moving violations in the last 3 years. No DWI, DUI, reckless driving, license revocation, or suspension in the last 2 years.

Residence/Citizenship –
Must be a permanent U.S. resident for the past 2 years and either a U.S. citizen or have permanent Visa or Green Card.
